Membership Terms and Conditions
~ Membership starts after first initial service
~ Valid ID and Credit Card
Club memberships are non-refundable and non-transferable. The benefits associated with an Uptown Haven Membership Wellness program include a monthly treatment (“Membership Treatment”), unique offers, promotions and retail product discounts.
~ Facials can rollover to the following month if you miss or cancel an appointment and facials are transferable to friends or family members.
~ The minimum, non-cancellable, term of a membership is three (3) months. After the minimum term, memberships will be automatically renewed for successive one month periods, unless and until the membership is canceled.
~ An automatic monthly payment will be charged on the 1st of every month.
~ Your membership must be active to receive exclusive benefits.
~ Membership fees may not be paid with Gift Cards or Certificates.
~ Appointments must be canceled at least 24 hours in advance, otherwise the appointment will be considered to have been used and an available Membership Treatment will be redeemed.
~ Your first treatment of the month is included in your monthly membership. Any treatments thereafter, will be paid by the client at a member discounted rate.
~ A cancellation fee of $50 will be applied if cancelled before the end of 3rd month term. All membership cancellations require a written notice by the 20th of the month.
~ Uptown Haven retains the right to terminate membership at any time, for any reason.
